Improving Continuity of Care by Strengthening Health Information Systems

This report includes case studies on the child protection information management system in Kenya and the gender-based violence referral system in Botswana. Both demonstrate the importance of multi-sectoral collaboration and key role of the social service workforce in providing continuity of care.
